I agree but both forms require you to sign a declaration and warn you that it is a criminal offence (two years imprisonment ) to make a false statement. It is up to you then if you want to do that.

The D4 also requires you to give your GP's details and you have the option to let them inform your GP and release reports to him, which I ticked.

Apart from the cost and convenience of this service, I felt I really didnt want to bother my GP with a bureaucratic procedure. My surgery is busy enough as it is and it can put them on the spot on how to answer some questions. This way it is down to me and a third party Dr who is dealing with these forms on a daily basis.

They didn't charge the dustbin lorry driverfor not telling the truth, did they?

Update. The new licence arrived today with all my old exemptions in place for next three years. Now to borrow a lawn mower so I can drive it over London Bridge

Last time I went over London Bridge they had brought it up to date with Tarmacadam®.

I noticed that there weren't as many horses and carts as there used to be either.
